You must use plan providers except in emergent or urgent care situations or for out-of-area renal dialysis. If you obtain routine care from out-of-plan providers, neight Medicare nor MD Care Inc. will be responsible for costs.

2. Read the following instructions and click on the Enroll Now button below.

• Clicking on Enroll Now will take you directly to the Centers for Medicare and Medicaid Services (CMS) Website.
• Please scroll down to the section titled SEARCH TOOL and select Compare Health Plans and Medigap Policies in Your Area.
• Follow the directions as instructed.
  
  

3. Make a home appointment by calling 1-888-285-9676 (TTY/TDD: 1-800-735-2929)

4. Request for information via mail by calling 1-888-285-9676 (TTY/TDD: 1-800-735-2929) 8am to 8pm, Monday through Sunday.

5. Download the MD Care Healthplan Membership Application Form (below) and mail to the address below:
